Before creating a new user profile, know that all Ohio physicians are already in OSMA's database, so there is likely a record of you on file.

Please start with "Search by Name" or "Search by License #" below. If you find that the email address we show for you is no longer valid, please call our office at 800-766-6762.

Search for Your Account by Name

Enter your last name and (optionally) any part of your first name to find your account in our records. If your search does not succeed at first, try leaving the first name field blank. Click the "This is Me" link beside the record that matches you and we will give you a hint about which email address we have on file for you and provide you tools to help you find your login.

If you need to recover (or create) your username click the "Forgot Username" link on the Sign In page and we will send it to the email address we have on file.
