Caring for your physical and mental health can help you stay at the top of your game as a healthcare provider. But often, the demands of practicing medicine and life in general can get in the way.  

The Well-Being Checkup And Referral Engagement Service is a simple, free, and anonymous checkup of your mental and emotional health. 

Take the first step in prioritizing your mental and emotional well-being by completing the voluntary, anonymous questionnaire designed to help you assess your current state of mental health and well-being.

Once completed, a licensed, master's level mental health clinician from the Ohio Professionals Health Program to review your questionnaire and provide you with a personalized response that includes recommendations for licensed, vetted mental health professionals, should you wish to take the next step. 

Your participation is completely voluntary and anonymous.

What happens after I take the questionnaire?

After you submit the Questionnaire, you will receive a personalized response via the Dialogue feature on this website, which will include information, recommendations, and options for next steps. 

Do I have to do anything with my recommendation?

No. What you do with your recommendation is completely up to you. You'll have the option of privately communicating with the licensed mental health professional (LMHP) through this website, on the phone or in person, or get a referral to a provider near you. Or you can decide to do nothing further at this time. No follow-up or service will be provided unless requested. 

Is my information tracked or known in reporting?

Your privacy is important. Therefore, your identity will not be known to anyone unless you decide to share it. Using this service is completely voluntary and anonymous.
